Telehealth Consent

Effective date: September 24, 2026

By using the CAPO Health platform you consent to receive healthcare services through telehealth from independent licensed providers of Arora Health & Aesthetics, LLC.

What telehealth means

Telehealth is the delivery of healthcare services using electronic communications — such as secure online questionnaires, messaging, and audio or video visits — when you and your provider are not in the same place.

What you understand and agree to

  • Your provider decides, in their independent clinical judgment, whether telehealth is appropriate for you and whether any treatment is appropriate. You may be referred to in-person care.
  • A telehealth evaluation has limits compared with an in-person visit, including the absence of a physical examination.
  • You will provide complete and accurate information about your health, medications and history.
  • Electronic communications are protected with safeguards required by HIPAA, but no system is free of risk.
  • Telehealth is not for emergencies. In an emergency, call 911.
  • You may withdraw this consent at any time by contacting us; withdrawal does not affect care already provided.
  • Your provider may need to be licensed in the state where you are located at the time of your visit.
